Have you made a University related purchase? Please use this form to submit your receipt/purchase information to caa-financial@uidaho.edu. If you do not have an itemized receipt (showing the cost/description of each item as well as any taxes or fees) you may be asked to submit a Lost Receipt Affidavit.
